When applying for supplemental security income benefits you will need many items such as your social security card, your birth certificate, information about your home such as your mortgage or your lease, payroll slips, bank books, insurance policies, and other information about your income and the assets that you own. Names, addresses, and telephone numbers of the doctors, hospitals, and the clinics you have been to for your disability are also required as well as proof of U.S. citizenship or eligible non-citizen status. Evidence of an account number at a bank or other financial institution is also helpful so that your benefits can be directly deposited into you account avoiding conflicts such as mail delays and theft.  Not providing the correct documents can delay your case by many weeks, another reason to seek out professional help when filing your claim.
