The Social Security Administration provides a checklist titled "Adult Disability Checklist" to help SSDI and SSI applicants gather information to complete their disability application. The checklist is primarily used for completing the online application but can also be useful for in-person or telephone interviews.
The checklist documents all relevant forms for the disability process: military service discharge information (Form DD 214), W-2 Form, Social Security Number(s), and checking or savings account numbers. Additional information which is needed by the SSA includes the names, phone numbers and addresses of the claimant's treating sources, the medication information for the claimant and the types of jobs the claimant has performed. Workers compensation information must also be provided when the claimant applies for SSDI or SSI benefits.
