Disabled children who are disabled from birth or up to the age of 18 may qualify for Supplemental Security Income benefits. Children must be considered disabled and have a severe physical or mental health condition which is expected to last at least 12 months or result in the child's death. The condition must "seriously limit the child's activities". The child must also have very limited income and resources to qualify for Supplemental Security Income. Parents who are applying for SSI benefits for their child must make an appointment, either on the phone or in-person, to complete a child disability report or application. Attending the appointment and bringing all the necessary information can ensure the application and interview are completed in a timely manner. Information which will be needed to complete the child disability report includes the following: • Medical information including a list of the doctors, hospitals and therapists who have seen the child for at least one year. Include the dates of the appointments, the child's ID numbers, medical records, medication and the child's medical assistance number. • Names, addresses and phone numbers of any schools the child has attended in the past 12 months. The names of teachers, psychologists, counselors, speech and other therapists who have provided special assistance for the child. • Information concerning a child's Individualized Family Service Plan (IFSP) for early intervention services or Individualized Education Program (IEP) for special education services (if these services have been needed to help the child). • An original or certified copy of the child's birth certificate or proof of U.S. citizenship or legal residency. • Full names and Social Security Numbers for the adults and children who live in the household. • Information about the income and resource level for the child and all the family members who live in the home this can include: pay stubs, self-employment tax returns, unemployment or other program benefits or child support.
